It can help pre-ignition knock, rough idling and in many situations - MPG.
This stuff did wonders for my SRT.

-1 can in a full gas tank
This will clean your fuel system from lines to injectors.

-Half a can into a vacuum line directly into your engine through a vacuum line, reattach line and turn off car, wait at least 10 minutes, start car in well ventilated area and run aggressively for 5 or so minutes. (video)
Cleans whole ventilation system from intake (depending what vacuum line you use) to cat.

-Half a can directly into your oil.
Do this just before an oil change - you should only drive about 50 miles with this, as your oil will become VERY dirty.
First time I did this on my SRT I drove about 50 miles and my oil looked like coffee.

-If I happen to have the intake off I'll use the spray to clean the throttle body and into the intake manifold.

All the smoke is the crap that had been building up inside the engine and exhaust.

EDIT: Also, I did it because it looks really cool. :)

Seafoam is also very handy if you happen to buy 80's era mercedes turbo diesels and plan on running them with filtered fryer grease.
